During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
A monitoring inspection and investigation was initiated on 1/24/25 and completed on 4/18/25 with a local agency in response to a self-report received by the licensing office on 1/24/25 relating to children's injuries. The inspector reviewed 1 children?s record and 1 staff record. This inspection included document review, a tour of the facility, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/28/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care and protection. On 1/23/25, child 1, age 3 years, told staff 1 multiple times during the afternoon hours that her arm was hurting. Staff 1 took no action to assess, treat or comfort the child nor did she inform center management that child 1 was complaining of arm pain. After picking her up from the center that evening, child 1?s parents took her for medical treatment and child 1 was diagnosed with a serious injury to her arm.

Plan of Correction: The center responded with the following: Closely watch, engage and redirect children to create safe learning opportunities. Training of staff will concentrate on active supervision and will include these six strategies: setting up the environment. positioning staff, scanning/counting, listening, anticipating children's behavior, engaging and redirecting. If an incident does occur, we will promptly assess, treat and comfort the child. The staff member involved will also complete an incident report. We will also re-train current staff (and train new staff) on incident reporting. Effective incident reporting can be achieved as long as you follow best practices. These practices include: complete incident reporting as promptly as possible; be thorough, clear and accurate as you report details; immediately inform the person in charge; and, be sure to include as many specific details as possible. If it is a serious injury that requires medical attention, it will be reported to the district leader, incident reporting, and to licensing as quickly as possible.
