An unannounced, on-site monitoring inspection was initiated on 04/09/2025 and completed on 04/15/2025. The on-site inspection began at 9:30 AM and ended at 11:30 AM. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 staff. The inspector reviewed 5 children?s records, 5 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The door frame that was accessible to children had several exposed screws.Plan of Correction: The administration has removed the door frame.
