During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
The Licensing Inspector discussed CPR/First Aid for new staff, liability of insurance, transportation, and ongoing training.

Comments:
An unannounced, on-site monitoring inspection was conducted 4/16/2025 from 9:35am - 12:05pm. There were 121 children and 22 staff present during the time of the inspection. The inspector reviewed 5 child and 5 staff records onsite. The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 4/29/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-140-A
Description: Each child shall have a physical examination completed by a physician before the child's attendance or within 30 days after the first day of attendance.

The record for Child #2, enrolled for 7 months, did not contain a physical examination.

Plan of Correction: Parents have been contacted to submit this form to the doctor.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled containing all required information.

The record for Child #3, enrolled for 16 days, was missing employment information for one parent and an address for the second emergency contact.
The record for Child #4, enrolled for 10 months, was missing employment information for one parent.

Plan of Correction: Parents have been contacted for missing information.

Standard #: 8VAC20-780-260-A
Description: The center shall provide to the licensing representative an annual fire inspection report.

A fire inspection had not been conducted since November 2023.

Plan of Correction: Fire inspection application has been submitted online.

Standard #: 8VAC20-780-260-B
Description: Annual approval from the health department shall be provided for meeting requirements for: water supply, sewage disposal system, and food service.

The health inspection conducted in 2024 was incomplete. The inspection did not include the annual approval for water supply and sewage disposal.

Plan of Correction: Health inspection was already done, a request for Environmental Sanitation has been mailed.

Standard #: 8VAC20-780-410-1
Description: REPEAT VIOLATION
The following acts are forbidden: roughly handling a child.

On 4/16/2025, Staff #1 was observed pulling Child #1 (age 4-5 years) by the forearm while walking down the hallway from the playground.

Plan of Correction: School staff are monitored for compliance with child interaction expectations, and on this date, the teacher was indeed guiding a child by the forearm, and not the hand, in an effort to prevent eloping. All staff have been notified of the need to refrain from guiding children in any manner that could be interpreted as a violation of 8VAC20/780-(6)-410-1. This was done via a staff meeting on 4/23/2025.
