An unannounced, on-site monitoring inspection was initiated and concluded on April 15, 2025. The on-site inspection began at 10:39 a.m. and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 64 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 29, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for child #3, enrolled for over two weeks, was missing the work phone number of each parent who has custody and the previous child day care attended by the child. Plan of Correction: All enrollment forms will continue to be reviewed by the administrative team at the time of enrollment to ensure every required field is either completed or properly notated if not applicable.
Standard #: 8VAC20-780-500-A Description: Staff shall wash hands with soap and running water after any contact with body fluids. Staff #3 did not wash hands after wiping mucus from a child's nose and the staff's arm. Plan of Correction: A hand hygiene refresher training was conducted for all staff that evening, emphasizing the importance of washing hands specifically after wiping a child's face.
Standard #: 8VAC20-780-550-F Description: Lockdown procedures shall be practiced at least annually. There was no lockdown procedure practiced in 2024. The most recent lockdown practiced was dated 9/13/23. Plan of Correction: Entire teaching and administration team was retrained on the proper place to document emergency preparedness drills.
