8VAC20-780 SPECIAL CARE PROVISIONS AND EMERGENCIES
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on 4/4/2025 and completed on 04/23/2025 in response to a complaint received by the licensing office on 3/31/2025 relating to allegations of special care provisions and emergencies. The on-site inspection was conducted by the inspector and administrator on 4/4/2025 and began at 11:10am and ended at 12:45pm. The inspectors reviewed compliance in the areas listed above. The inspectors reviewed 1 child record and 2 staff records, one on-site. This inspection included document review, tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/30/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-130-E Complaint related: No Description: The center is required to obtain documentation of additional immunizations once every six months for children under the age of two years.
Child 1, who had been enrolled for over a year, had no updated immunizations in the records.Plan of Correction: Mom for child #1 has updated her immunizations

The center shall maintain a written record of children?s serious and minor injuries in which entries are made the day of occurrence.
An incident report was created 6 days after an injury occurred. The report was also missing the time of occurrence and future action to prevent recurrence of the injury.Plan of Correction: The incident report was actually written the day it happened and was left in the classroom. The AD was asked on Wednesday several days later by the mom that she did not receive the report. Teachers had assumed mom had taken it however when they went back to look for it, it was missing. They did write one up on Wednesday when it could not be found. The PD will ensure that all incidents? reports are signed the day of and given back to the office.
