During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 04/24/2025, as a part of the licensure period. The on-site inspection began at 11:30a.m.and ended at 1p.m. The inspector reviewed compliance in the areas listed above. There were23 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
- Violations:

Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
One area of the center had peeling paint which was accessible to children under the age of 18 months.Plan of Correction: A maintenance request will be made to sand and repaint this area.
Standard #: 8VAC20-780-520-A Description: Over-thecounter skin products shall not be kept or used beyond the expiration date of the product.
One topical ointment expired over 1 1/2 years ago. One topical ointment expired 9 months ago.Plan of Correction: The two ointments will either be disposed or returned to the parents.
Standard #: 8VAC20-780-570-B Description: Bottle fed infants who cannot hold their own bottles shall be held when fed.
An infant who is unable to hold their own bottle was not held by staff while being fed.Plan of Correction: Staff immediately picked the child up from the bouncy seat and held the child while bottle feeding.
Standard #: 8VAC20-780-570-E Description: Prepared infant formula shall be dated.
Three bottles belonging to Child #6 were not dated.Plan of Correction: Bottles were dated.
