Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on April 30, 2025. The on-site inspection began at 11:05am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 14 staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included document review, tour of the facility, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.011-F
Description:	The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 11/12/2024, were not posted in a conspicuous place on the premises.
Plan of Correction:	The director will ensure the inspection summary and violation notice are posted within 24 hours of receiving the inspection reports.
Standard #:	22.1-289.035-A
Description:	The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #3 was overdue by seven months. The most recent national fingerprint check for staff #3 was overdue by seven months.
Plan of Correction:	We will schedule an appointment for the fingerprints and will submit the request for the central registry search.
Standard #:	8VAC20-780-140-A
Description:	Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been enrolled for seven months.
Plan of Correction:	Family Service Manager and the Health Manager will conduct monthly monitoring of the children's files. We will reach out to the physician for the most current physical documentation.
