During the inspection, the inspector reviewed the areas listed above to include standards found
out of compliance during the previous inspection. Unless otherwise noted as a violation within
this inspection report, the provider was in compliance with the standards reviewed. If there were
any serious injuries or fatalities related to a violation, the details will be included in the
description of the violation.

An unannounced, on-site renewal inspection was initiated on 04/25/2025 and completed
on 04/25/2025. The on-site inspection began at 12:30pm and ended at 2:14pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 4 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included
document review, tour of the facility, interviews, observations and measurements .
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 8, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.057-A Description: The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan. Plan of Correction: The necessary department has been notified to perform this testing.
Standard #: 8VAC20-780-80-A Description: Repeat Violation
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The center did not have written record of departure of each child when it occurs.Plan of Correction: The daily attendance book has been updated to include the departure of each child.
Standard #: 8VAC20-780-245-L Description: There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
There was no staff on duty that had daily health observation training in the past three years.Plan of Correction: Staff has been instructed to renew / update their health and safety orientation.
