During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 4/24/2025, as a part of probation. The Department issued a Special Order on 1/17/25, to place the license on probation for a period of six months and require the licensee to contact parent, guardians or other responsible persons (families) regarding health and safety violations, as a result of high-risk violations. The on-site inspection began at 10:35am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 92 children present and 17 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, observation, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation nocte issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/13/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A
Description: Repeat violation
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record for Child 4 who had been enrolled over 30 days.

Plan of Correction: Child #4 has returned the physical

Standard #: 8VAC20-780-150-B
Description: Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.

The immunization record for Child #4 was not signed.

Plan of Correction: PD Child #4 has returned the signed immunizations

Description: Repeat violation
The center shall ensure that each grouping of children has a qualified program lead.

Staff 5 was working as a program leader on the day of the inspection. Staff 5 did not have documentation of the educational requirements for a program leader position.

Plan of Correction: PD will ensure that staff that are program lead has all documentations

Description: Cribs, cots, rest mats, and beds shall be identified for use by a specific child.

In the younger infant room, children were in cribs not identified for use by that specific child.

Plan of Correction: Cribs have been labeled and reviewed with teachers.

Description: Repeat Violation,
Staff shall wash hands with soap and running water before and after a diaper change
Staff did not wash their hands after changing a diaper.

Plan of Correction: PD has required the teachers to take the handwashing training again.

Description: Repeat Violation,
The diapering surface shall be cleaned with soap and water and sanitized after each use.

Staff cleaned the diapering surface with a sanitizer only.

Plan of Correction: PD has reviewed with staff on sanitizing after changing a diaper

Description: The record of each child on formula shall contain the child?s feeding schedule.

There were no feeding schedules for the children in care.

Plan of Correction: PD has spoken with Infant teachers that all infants must have feeding

Standard #: 8VAC20-780-570-E
Description: Prepared infant formula shall be refrigerated, dated and labeled with the child?s name.

In the infant 1 & infant 3 class, there were multiple bottles in the refrigerator that had no date.

Plan of Correction: PD has spoken with Infant teachers to remind them that all bottles must be dated
