Technical Assistance:
1. COVID-19 health + safety guidance reviewed with the vendor, and discussed additional safety measures she has chosen to require at her home.
2. There should be no blank spaces on child enrollment forms. If there is no information to list the parent should write "N/A" in the space on the form.
3. Daily attendance records do not require a signature. The vendor may record it without the parent signing it.
4. The inspector contacted Fairfax County to request assistance for the vendor in resolving payment issues with the food program; and the State in resolving child care subsidy payment issues.

Comments:
This inspection was conducted by staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time.

An unannounced subsidy inspection was conducted on 9/25/20 with the assistance of a Spanish translator. The vendor was contacted by phone to initiate the inspection, and emailed a list of items required to complete the inspection. A Spanish speaking inspector assisted with the translation of written documents. There were 2 children present = 1 point with the vendor. The vendor reported that the daily activities are mostly virtual school. She will take them outside for walks when the weather is nice. The vendor is currently participating in the USDA Food Program. Information gathered during the inspection determined there were non-compliances with applicable standards or laws reviewed. Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 1 of 2 child records reviewed did not include required information.

Evidence:

Child #4 -

1. There was no home address for 1 of 1 parents listed, and on the form an address written appeared to be covered in white-out.

2. There was no home work addresses/phone numbers listed for the 2nd parent, and the space on the form for this information was left blank.

Plan of Correction: The vendor will ask the parent to complete the form.

Standard #: 22VAC40-665-210-B
Description: Based on records reviewed, it was determined that 1 of 2 child records did not include immunizations once every 6 months until the child is 2 years old. Evidence: Child #4 - The immunizations were dated June 2011, when the child was 12 months old.

Plan of Correction: The vendor will ask the parent for updated immunizations.
