During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 5/1/2025, as a part of the licensure period. The on-site inspection began at 11am and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 98 children present and 17 staff. The inspector reviewed 9 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The documentation of immunizations in the record for Child #8, who had been attending for five months, were obtained three months after Child #8 entered care.

Plan of Correction: Immunizations were obtained for the child. The center will obtain immunization documents before children enter care.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. The physical examination in the record of child #8, who had been enrolled for five months, was obtained three months after Child #8's first day of attendance.

Plan of Correction: A physical was obtained and placed in the child's record. The center will ensure physicals are obtained within 30 days of a child entering care.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, enrolled for three months, was missing the work phone number and place of employment for one parent.

Plan of Correction: The center will request the information from the parent.

Standard #: 8VAC20-780-60-A-8
Description: A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #4, who has been enrolled for three months, has a diagnosed food allergy. The written care plan in the record for Child #4 did not contain instructions from a physician regarding steps taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The center will request from the parents an allergy action plan with instructions from the child's physician.

Standard #: 8VAC20-780-550-P
Description: The center is required to maintain a written record of children's serious and minor injuries which include all required information. 3 injury reports reviewed did not contain the date and time when parents were notified.

Plan of Correction: The center will review requirements for injury reports with staff.
