During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 4/16/25 and completed on 4/23/25, as a part of the licensure period. The on-site inspection began at 10:45am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 75 children present and 16 staff. The inspector reviewed 5 children?s records on-site and 5 staff records electronically on 4/23/25. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/13/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-440-J
Description: Thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. There was not 30 inches of space between two occupied cribs.

Plan of Correction: Administration redesigned the classroom to ensure 30 inches of space in between occupied cribs.

Standard #: 8VAC20-780-440-L
Description: Pillows and filled comforters shall not be used by children under two years of age. An infant in care was sleeping in a fiber filled sleep sack.

Plan of Correction: Staff were retrained on reading labels on sleep sacks. Parents received handout regarding requirements of sleep sacks.

Standard #: 8VAC20-780-500-B
Description: The diapering surface shall be used only for diapering. The diapering surface was used as for storage of diaper bins in two classrooms.

Plan of Correction: Staff were trained to return all bins to storage area prior to changing all children. Staff were advised to take out diaper and wipes at changing time.

Standard #: 8VAC20-780-570-E
Description: Prepared infant formula shall be dated. There were bottles dated 4/14 and 4/15, one each. However, the current date was 4/16. Additionally, there were 15 infant bottles that were not dated as required.

Plan of Correction: Staff were retained on bottle labels. Parents received another handout regarding requirements for bottle labeling and sent individual messages regarding the importance of the dates on the bottle.
