Comments:
An unannounced, on-site monitoring renewal inspection was initiated on 05/07/2025 and completed on 05/08/2025. The on-site inspection began at 10:00 am and ended at 11: 55 am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children?s records, 4 staff records, and 1 applicant record on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening.

The updated TB screening for Staff 1 was due 20 months ago. The updated TB screening for Staff 3 was due 16 months ago.

Plan of Correction: Will get these done this week.

Standard #: 8VAC20-780-245-A
Description: Staff shall complete a minimum of 16 hours of training annually.

Staff 3 obtained 11 of the 16 required hours in 2024.

Plan of Correction: I will assign the staff 5 more hours of training by Friday, May 16 and plan 16 hours going forward.

Standard #: 8VAC20-780-560-G
Description: REPEAT VIOLATION

When food is brought from home the food container must be clearly dated.

None of the children?s lunchboxes were dated.

Plan of Correction: Will ensure they are dated each day.
