During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 5-6-2025 and completed on 5-8-2025 . The onsite inspection began at 2:30pm and ended at 5:30pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 10 staff. The inspector
reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews,observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/15/2025.
A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-M
Description: (Repeat Violation)
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The list in 4 rooms was not dated and in 1 room it was not kept in a confidential manner.

Plan of Correction: Center will ensure that the allergy list will be put away at all times for confidential

Standard #: 8VAC20-780-60-A
Description: (Repeat Violation)
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
1.Child #1?s record did not include the appropriate legal paperwork on file when the custodial parent requests the center not to release the child to the other parent.
2.Child #1, Child #3 and Child #5?s records did not include the annual update. Child #1?s record was overdue by more than 9 months, Child # 3?s was overdue by more than 1 ? years and Child #5?s was overdue by 8 months.

Plan of Correction: Center will ensure that each record will be updated yearly with any changes

Standard #: 8VAC20-780-280-B
Description: Hazardous substances must be in a locked place that prevents access by children. In Classroom A and Classroom C a soap solution labeled keep out of reach of children was observed in an unlocked manner.

Plan of Correction: Center will ensure soap solution will be locked away in the lock box at all times

Standard #: 8VAC20-780-330-B
Description: (Repeat Violation)
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus. The resilient surfacing around the spring bouncers (truck and boat) and both meri-go-cycles was insufficient as it measured between 1 inch to 4 ? inches and a minimum of 6 inches is required.

Plan of Correction: Center will ensure that resilient surfacing shall comply with state standards with 6 inches requirement.

Standard #: 8VAC20-780-330-C
Description: Ground supports shall be covered with materials that protect children from injury. The metal supports for the spring bouncers (truck and boat) had protruding bolts and were not covered in a way to protect children.

Plan of Correction: Center will ensure all playground equipment will be cover by mulch according to state standards

Standard #: 8VAC20-780-520-C
Description: If diaper ointment or cream is used these products do not need to be kept locked but should be inaccessible to children. In Classroom A one diaper cream was observed on the shelf under the changing table and was accessible to children.

Plan of Correction: Center will ensure to keep diaper ointment cream

Standard #: 8VAC20-780-560-F
Description: Centers shall follow the most recent, age-appropriate nutritional requirements of the Child Care Food Program of the United States Department of Agriculture (USDA) which requires two food groups shall be served as snack and must meet the required nutritional guidelines. The snack provided, Frito Lay chips and Fudge Rounds cookies, do not meet the nutritional guidelines and 2 of the 5 food groups we not served.

Plan of Correction: Center will follow the USDA requirements that are provided for snacks USDA program
