8VAC20-780 Staffing and Supervision
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on 4/8/2025 and completed on 04/11/2025 in response to a self-report received by the licensing office on 4/3/2025 relating to staffing and supervision. This inspection included, document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 04/02/25, Child #1, four years old, was left in the classroom asleep for approximately 13 minutes.Plan of Correction: Staff #1, #2, and #3 were terminated. Beginning April 3, 2025 and continuing each afternoon that bus service is provided, procedures for emptying classrooms and loading buses have been modified as follows:
1 ? Children line up in classroom. Teacher calls roll and verbally counts children. Aide verbally counts children and compares count to teacher?s count and to the number on the classroom child count board ensuring that all children are accounted for.
2 ? Children exit the classroom and line up in hallway. Teacher calls roll and verbally counts children again.
3 ? Third staff member immediately enters classroom and performs classroom sweep to ensure that no children have been left in room.
4 ? Once all children are accounted for and third person has verified that no children remain in classroom, children are moved into bus lines.
5 ? Once children are in bus lines, bus monitor calls roll from bus roster and verbally counts children. If a child rode the bus in the morning but is not present for the afternoon route, the bus monitor will verify with the child?s teacher (while in the hallway and staff members are together) why the child is not present.
6 ? Once bus roll is called and children are accounted for, the bus driver will lead children to the bus with the bus monitor following.
7 ? Once on the bus, the bus monitor will again call roll and count children.
8 ? After hallway clears, a staff member not on the buses (generally the building cook) will check all restrooms and classrooms again to ensure that no children have returned to these areas.
9 ? This staff member will indicate that all restrooms and classrooms are clear to the lead bus driver before buses leave the center.
Additionally, program active supervision policies and procedures, program expectations, and standards of conduct will be reviewed with all program staff (from this center as well as all other program HS/EHS centers) on April 21, 2025 at the program?s scheduled staff meeting.
Standard #: 8VAC20-780-340-F Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On 04/02/2025 staff #1 and staff #2 lined up the classroom to load the bus. The total # of children in class was 11. Child #1 was left in the classroom asleep. Once the children were loaded on the bus, staff #3 did not take the role. Staff #4 did a sweep of the classroom and found child #1.
