An unannounced inspection was initiated on 05/06/2025 and completed on 05/09/2025 in response to a complaint received by the licensing office on 04/23/2025 relating to allegations of staffing and supervision.
The inspection included interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/16/2025. A POC submitted after this date will not appear on the public website.

When children are in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group.
On 04/22/2025, an infant class and a preschool-aged class were combined for approximately one hour. There were between 16-18 children in the care of 2 staff members during this period of time. The staff-to-child ratio for children from birth to 16 months (1 staff:4 children) was applicable to this group of children.
