Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 5/6/25. The on-site inspection began at 1:45pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 60 children present and 9 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/19/25. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-4
Description:	The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record for staff #1, employed for 3 months, did not have documentation of a sex offender registry check for a state identified on the sworn statement. The record for staff #2, employed for 5 weeks, did not have documentation of a sex offender registry check for a state identified on the sworn statement.
Plan of Correction:	The facility conducted a search of the sex offender registry for each staff member during the inspection.
Standard #:	8VAC20-780-40-M
Description:	Repeat violation The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The list in the waddler room was not dated.
Plan of Correction:	The facility responded: Took the allergy list that did not have a date and added one that was dated.
Standard #:	8VAC20-780-270-A
Description:	Repeat violation The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. A toilet seat in a girls bathroom was pulling out of the base. The seat was loose. There was peeling paint on the wall in the boys bathroom.
Plan of Correction:	The facility responded: Reminded staff to report hazards to management immediately. Placed a work order for paint and toilet.
Standard #:	8VAC20-780-280-B
Description:	Repeat violation Hazardous substances must be in a locked place that prevents access by children. In a preschool classroom, there were 3 deodorizers labeled "keep out of reach of children" placed throughout the classroom in areas accessible to children. There were cleaning and sanitizing solutions in an unlocked cabinet. In a bathroom shared by two classrooms there were cleaning and sanitizing solutions in an unlocked closet.
Plan of Correction:	The facility responded: Retrained the teachers on classroom procedures according to licensing standards to ensure hazardous substances are locked away and out of the reach of children.
Standard #:	8VAC20-780-550-G
Description:	Repeat violation Documentation shall be maintained of emergency evacuation drills that includes all of the required information. The emergency evacuation documentation for November did not include the time and number of children participating in the drill.
Plan of Correction:	The facility responded: Will ensure that moving forward, documentation for emergency evacuation drills are filled out completely.
