During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated and completed on 5/7/25 with a local agency in response to a self-report received by the licensing office on 5/2/25 relating to supervision. The inspector reviewed 1 children?s records and3) staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/19/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The Center is required to ensure that the centers activities, services and facilities are maintained in compliance with their own policies that are required by the standards.
The center did not follow their written playground safety procedures that included a name to face check when the children line up to come inside from the playground, when sending students inside from the playground, and after all students enter the classroom which also included a final visual sweep of the playground before closing the playground door. On 5/2/25, staff stated they did not follow their playground policy and Child #1 (age 20 months) was left on the playground alone.Plan of Correction: All staff have been re-trained
in playground safety policies
and procedures. In addtion, prior to
classrooms transitioning from
playground to class or class
to playgrounds, they are supervised
by a management team member to
ensure name to face procedures are
followed and that
all students transition safely from one
area to another.
Standard #: 8VAC20-780-340-F Description: The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.
On 5/2/25, the Toddler classroom was on the playground, and when transitioning to inside, Child #1 was left outside for approximately 60-90 seconds while the rest of the class and teachers went indoors. A staff member from another classroom discovered the child on the playground, crying and alone.Plan of Correction: All staff have been re-trained
