Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed that there shall be twelve inches of space between the sides and ends of occupied cribs.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on May 7, 2025. The on-site inspection began at 8:30am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 25 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.011-F
Description:	The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 07/02/2024, were not posted in a conspicuous place on the premises.
Plan of Correction:	I was certain it was posted, but I will ensure the most recent inspection is posted going forward. The violation was corrected during the inspection.
Standard #:	8VAC20-780-130-E
Description:	The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #3, age 21 months, did not have documentation of additional immunizations once every six months. The last documentation on file was dated over seven months ago.
Plan of Correction:	We will reach out to the parents today to request they bring their child's updated immunization record as soon as possible.
Standard #:	8VAC20-780-440-B
Description:	Cribs shall be identified for use by a specific child. One crib was not label with the child's name.
Plan of Correction:	We had moved a child to another classroom on Monday, and the new child started on Monday. Also, we had moved our classrooms around, and just had not added the new child's name to the crib. It will be labeled today.
Standard #:	8VAC20-780-550-D
Description:	The center shall implement a monthly practice evacuation drill. A practice evacuation drill was not conducted for January 2025.
Plan of Correction:	We didn't realize we had missed the month of January. We will ensure the practice drills are completed as required.
