During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated and completed on 5/8/25. The Department issued a Notice of Intent to place the licensee on probation for a period of six months; and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 3/12/25 as a result of high-risk violations. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/19/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-770-60-C-2 Description: (Repeat Violation) The Center is required to obtain documentation that a central registry search was completed by the end of the 30th day of employment.
The record for Staff #1 contained documentation that a central registry search was completed 10 months after hire. No documentation of follow up was in the file.Plan of Correction: Staff #1 's central registry check was completed on 3/11/2025. Going
forward, no new staff will start until it has been completed and
returned.
Standard #: 8VAC20-780-160-A Description: (Repeat Violation) The Center is required to obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children and within 30 days prior to employment that is signed by a physician or a physician?s designee.
The file for Staff #1 contained a TB test or screening that was completed 21 days after hire.Plan of Correction: Staff #1 's TB test was acquired on 6/24/2024. Going forward, no new hire will start without a TB test.
Standard #: 8VAC20-780-240-E Description: (Repeat Violation) The Center is required to ensure that staff complete a First Aid and CPR orientation appropriate to the age of the children in care within 30 days of the first day of employment.
The file for Staff #1 did not contain documentation of a completed First Aid and CPR orientation within 30 days of employment.Plan of Correction: Staff #1 has completed CPR training 3/13/2025. Going forward, an new staff members will receive CPR
orientation on their first day of
employment.
Standard #: 8VAC20-780-350-B-4 Description: The Center is required to maintain the required supervision ratio of 1 staff to 10 children whenever children are in care for children aged 3 years old to school age eligible.
During inspection in the TK classroom, Staff #2 was alone with 12 children (age 5 years old) when Staff #3 was in the eating area divided by the half wall.Plan of Correction: All team members have been instructed to view each side of the room as two different classrooms and will remain on one side only.
