During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site focused inspection was initiated and completed on 5/7/2025, as a part of pending enforcement action. The on-site inspection began at 10:40am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 10 staff. The inspector reviewed 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/21/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: The center is required to request a child abuse and neglect central registry search prior to employment from any state in which staff has resided in the preceding five years.
The center did not request a search of the child abuse and neglect registry from one state for Staff #3, who has been employed for more than three weeks and was working alone with children at the time of the inspection.Plan of Correction: Not available online. Standard #: 8VAC20-780-160-A Description: Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with the children.

Areas and equipment inside and outside of the center shall be maintained in a clean and safe condition.
1. On the "little playground," the mulch used as resilient surfacing in the fall zones of the slides and climbing equipment consisted of long splintered wood pieces measuring five to six inches in length posing a scratch and impalement hazard.
2. In the Toddler room, a section of cove base was detached from the wall by the exterior door exposing a layer of dirt, crumbling drywall, and peeling paint.
3. On the "big playground", one set of double gates had a missing latch or lock and the gate was unable to be secured. The gates were ajar leaving a large gap and children were observed pushing on the gates.Plan of Correction: Not available online. Contact Inspector for more information.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Standard #: 8VAC20-780-330-B Description: Resilient surfacing shall be underneath and in the fall zones of the outside play equipment with moving parts or climbing apparatus.
On the "big playground", the double green slide, approximately three and a half feet in height, had grass growing four feet into the fall zone. At the base of these slides there was dirt showing through a thin layer of mulch. At the base of the curved green slide, measuring three and a half feet in height, there was only one inch of resilient surfacing. At the base of the taller green slide, measuring four feet in height, there was only one inch of resilient surfacing. Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.
The Toddler classroom has been without a qualified lead teacher for over six months. The 2's classroom has been without a qualified lead teacher for over two months. Standard #: 8VAC20-780-400-B Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Staff #4 was heard loudly yelling "stop bothering them!" from down the hallway. Staff #4 then stated to a child "color the paper or you won't color at all" with a raised voice and harsh tone and slammed a colored pencil down on the table in front of the child. Later, this same staff member was speaking to a child attempting to go across the monkey bars on the playground and stated "Come on, if they can do it you should be able to do it, why can't you do it?" with a taunting tone.Plan of Correction: Not available online.
