Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 05/12/2025, as a part of the licensure period, The on-site inspection began at 3:30 PM and ended at 4:55 PM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included: ? document review, ? tour of the facility, ? interviews, and ? observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-130-A
Description:	Repeat violation The record provided for child 1 did not contain a record of immunizations. The immunization records in the record provided for child 2 were not signed by a physician.
Plan of Correction:	Provider response Children will not be allowed to attend without a copy of immunization records signed by a physician.
Standard #:	8VAC20-780-140-A
Description:	Repeat violation The record provided for child 2 did not contain the results of a physical examination.
Plan of Correction:	Provider response: All children's records will be reviewed to ensure that all children's records have the results of physical examinations.
Standard #:	8VAC20-780-60-A
Description:	Repeat violation Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the parents place of employment and business telephone. The record for child 3 contained the father's place of employ but no business telephone number.
Plan of Correction:	Provider Response Children's records will be reviewed to ensure that all required information is documented.

Description:	The center shall maintain a written record of children's minor injuries that includes the time the parent was notified of the child's injury. A minor injury record from January 2025 for child 3 did not contain the time the parent was notified of the child's injury.
