An unannounced, on-site monitoring inspection was completed on 05/02/2025, as a part of the licensure period. The on-site inspection began at 12:00 pm and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 staff. The inspector reviewed 5 children?s records and 2 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-M Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
There was no allergy list in the preschool room and there are children in that room with allergies.Plan of Correction: Center will have an allergy list in each area where children are.
Standard #: 8VAC20-780-260-B Description: After the first license, annual approval from the health department shall be provided.
There was no annual inspection from Health Department.Plan of Correction: Center will contact Health Department and request an inspection.
Standard #: 8VAC20-780-340-C Description: During the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.
There was only one staff member present during this inspection.Plan of Correction: Center will have two staff on site at all times.
Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
There was no evacuation drill recorded for April 2025.Plan of Correction: Center will practice and document evacuation drills monthly.
