During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on May 12, 2025, as a part of probation and an Intensive Plan of Correction (IPOC). The Department issued a Special Order on April 15, 2025, to place the licensee on probation for a period of six months; and contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The Department requested that the provider complete an IPOC as result of the last inspection, related to building maintenance, supervision of children, behavioral guidance, forbidden actions, handwashing and toileting procedures and background checks.
The on-site inspection began at 10:53am and ended at 1:48pm. The inspector reviewed compliance in the areas listed above. There were 47 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 21, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: Repeat Violation and Systemic Deficiency
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Four cots used for nap time in classroom #1 were torn and fraying.Plan of Correction: The cots with the frayed mesh netting have been removed from use and replacements ordered.
Standard #: 8VAC20-780-440-L Description: Pillows and filled comforters shall not be used by children under two years of age.
Child #1 (age: 8 months) was observed sleeping on a boppy pillow on a classroom mat in classroom #2. The tag on the boppy pillow states "Do not use for sleeping". Staff #1 and Staff #2 stated that the child does not sleep in a crib and regularly sleeps on the boppy pillow.Plan of Correction: Child #1 fell asleep shortly after being fed and the staff did not want to interrupt or withhold rest from the child by moving them. Staff have been apprised that moving a child who falls asleep on a boppy pillow will no longer be considered interrupting or withholding rest and will attempt their best to move the child to a crib without waking them.
