An unannounced, on-site monitoring inspection was initiated on May 6, 2025 and completed on May 6, 2025. The on-site inspection began at 10:35am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/20/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-C Description: At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. The tuberculosis screening in the record for staff #1 was expired for approximately one month. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was a an unlocked cabinet in the children?s restroom that had a container of disinfectant wipes and a can of disinfectant spray.
