An unannounced, on-site monitoring inspection was initiated and completed on 5/7/2025, as a part of the licensure period. The on-site inspection began at 10:40am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-150-B Description: Immunization records for each child must be signed by a physician, his designee, or an official of a local health department. The immunizations in the record for Child #3, who was in care and has been attending the center for more than 5 months, did not contain a signature by a a physician, his designee, or an official of a local health department. Plan of Correction: The center will request documentation of immunizations signed by the child's doctor.
Standard #: 8VAC20-780-70 Description: The center must maintain a record for each staff that contains all required information.
The record for Staff #3 did not contain the name, address, and telephone number of a person to be notified in an emergency.Plan of Correction: Corrected during inspection.
