During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 05/13/2025 and completed on 05/13/2025. The on-site inspection began at 9:11am and ended at 11:56am. The inspector reviewed compliance in the areas listed above. There were 44 children present and seven staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, observations, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/03/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1
Description: Repeat Violation:
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

Staff #5, employed for two weeks, did not have a completed sworn statement. Staff was working at the time of inspection.

Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #1, who has been employed for three months, did not have a completed national criminal background check. Staff #5, who has been employed for two weeks, did not have a completed national criminal background check. Staff #1 and staff #5 were working at the time of inspection.

Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Description: Repeat Violation:
Each child shall have a physical examination by or under the direction of a physician before the child's attendance; or within 30 days after the first day of attendance.

The record of child #4, enrolled for 15 months, did not have documentation of a physical examination.

Description: Repeat Violation:
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Staff #5 had no documentation of a tuberculosis screening and had been employed for two weeks.

Description: The center shall meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.

There was no documentation of viewing proof of the identity and age of Child #4, enrolled for 15 months, at the time of the inspection.

Description: The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #3, employed for seven months, did not have the Virginia Department of Education-sponsored orientation course complete. Staff #4, employed for nine months, did not have the Virginia Department of Education-sponsored orientation course complete.

Description: Repeat Violation:
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

The medication authorization for child #6 expired over two months ago. The medication was still on site.
