An unannounced monitoring inspection was initiated on 05/06/2025 and completed on 05/12/2025, in response to a self-report received by the licensing office on 05/06/2025 relating to programs. The inspector reviewed 1 child?s record and 1 staff record on-site. The inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/22/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Description: The following actions or threats thereof are forbidden: roughly handling or shaking a child.
On 05/06/2025, Staff 1 did not ensure that forbidden actions were not used when they roughly handled Child 1 (age 2 years). Child 1 was picked from a seated position on the ground by their left arm; the child was lifted to a height that did not allow their feet to touch the ground.Plan of Correction: Staff 1's employment has been terminated according to EHS center code of conduct policy. Additionally, Education Manager will develop and implement mandatory training for center staff on proper teacher-child interactions, appropriate redirection, and understanding differences between home care and programmatic care.
