An unannounced, on-site monitoring inspection was initiated and completed on 05/07/2025. The on-site inspection began at 3:15 p.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 18 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The records for Child 2, Child 3 and Child 4 enrolled over seven months, were each missing several of the required components.Plan of Correction: Staff will obtain info from parents.
Standard #: 8VAC20-780-260-B Description: Annual approval from the health department shall be provided.
The annual health inspection was overdue by 1 year and 1 month.Plan of Correction: Inspection was obtained.
