An unannounced, on-site monitoring inspection was initiated on 5/12/2025 and completed on 5/15/2025, as a part of the licensure period. The on-site inspection began at 11:47AM and ended at 12:43PM. The inspector reviewed compliance in the areas listed above. There were 9 children present and 6 staff. The inspector reviewed 3 children?s records on site on 5/12/2025 and 4 staff records electronically 5/15/2025. Medications were reviewed on site 5/12/2025. This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
