Comments:
On 8/31/20 a virtual focused inspection was conducted. Due to the Covid19 pandemic the inspector was not on site. This inspection was unannounced and conducted via mobile video application as well as phone and email correspondence. During the virtual inspection 2 children were present observed playing freely. The home and emergency supplies and procedures were reviewed. Vendor and household member background checks were reviewed provided by local jurisdiction. Violation were indicated in the violation notice.

Standard #: 22VAC40-665-170-B
Description: Based on the review of child records it was determined that all required information was not maintained.

Evidence: During inspection: Child Records, numbers 1, 2, 3, and 4 were missing proof of identification.
Child record 3 was missing parent work work information

Plan of Correction: Vendor stated that she will contact her local worker to get the proper paperwork to document proof of child identification and would get the information from parent.
