Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced inspection was initiated and completed on 05/06/2025 in response to a complaint received by the licensing office on 04/17/2025 relating to allegations of operational responsibilities and nutrition and food services. The inspector reviewed 1 children?s record and 3 staff records on-site. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 05/26/2025. A POC submitted after this date will not appear on the public website.

Description:	The operational responsibilities of the licensee shall include ensuring that the centers activities, services, and facilities are maintained in compliance with the centers own policies and procedures that are required by the standards. 1. The centers allergy policy states "for health and safety purposes, all staff members must monitor the Allergy & Dietary Preference List prior to every meal, snack time, or using food products of any kind. 2. Interviews and review of documentation revealed that child #1 was served food containing ingredients that the child has a documented food allergy to.
Plan of Correction:	Staff coached and retrained on all policies and procedures based around allergy and dietary preferences. Including but not limited to checking the allergy and dietary preferences list prior to serving all meals to children. Ensuring that a child with an allergen or dietary preference has a meal substitute.

Description:	A child with a diagnosed food allergy shall not be served any food identified in the written care plan. Child #1 has a diagnosed food allergy that requires the use of an epi-pen. Interviews and review of written documentation revealed that child #1 was served food identified in the written care plan on two separate occasions.
Plan of Correction:	All staff retrained on all policies and procedures based around allergy and dietary preferences.
