During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Technical assistance was provided and standards reviewed in the following areas: 8VAC20-780-270. Building maintenance.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 5/13/25. The on-site inspection began at 11:30am and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. There were 68 children present and 13 staff. The inspector reviewed six children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/27/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment.
The central registry search for Staff #1 (employed for three months) was not requested before the employee's first day of employment, which is required.

Plan of Correction: Effective immediately, the center will ensure that a Central Registry background check is requested for all prospective employees prior to their first day of employment. Management will be responsible for initiating and documenting the request for the Central Registry search during the hiring process prior to day one of employment. A copy of the request confirmation will be placed in the employee?s personnel file. Monthly internal audits will be conducted to ensure compliance with this procedure.

Standard #: 8VAC20-780-150-B
Description: Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
The immunizations for Child #1 (enrolled for one year) and Child #2 (enrolled for two years) were not signed by a physician, his designee, or an official of a local health department.
The physical exam for Child #1 (enrolled for one year), Child #2 (enrolled for two years) and Child #3 (enrolled for one month) was not signed physician, his designee, or an official of a local health department.

Plan of Correction: Effective immediately, the center will ensure that all physical examination and immunization reports submitted for enrollment are either electronically signed, physically signed, or stamped by a licensed physician, their designee, or an official from the health department. All submitted health forms will be reviewed by management upon receipt. Any forms that are incomplete or missing appropriate signatures or stamps will be returned to the parent/guardian for correction before the child may attend. Families will be informed at enrollment and during annual health record updates that all physical and immunization records must meet documentation requirements, including proper signatures or official stamps. Monthly audits of children's files will be conducted to ensure all health documentation is current and properly verified.

Standard #: 8VAC20-780-290-A-2
Description: Fans, when used, shall be out of reach of children.
There was a fan located next to a crib in an infant room. The fan was accessible to the infant in the crib.

Plan of Correction: Effective immediately, all fans used within the center will be placed out of reach of children to prevent any risk of injury. All standing or tabletop fans currently in use will be repositioned to locations where they are securely out of children's reach. Staff will perform daily classroom safety checks to ensure that fans remain properly positioned and are not accessible to children throughout the day. Management will conduct daily walk-throughs of all classrooms to verify proper placement of fans and adherence to safety protocols.
