Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 5/19/2025. The on-site inspection began at 12:00pm and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 53 children present, 8 staff, and 8 co-op parents. The inspector reviewed 5 children?s records, 4 staff records , and 2 co-op parent records on-site on 5/19/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: At least every two years from the date of the first initial screening or testing staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Staff #1 and Staff #2 both had TB screenings on file that expired two weeks ago.

Plan of Correction: Corrected during the inspection and new TB screenings were placed in the files of both staff members.
