Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed requirements that for children younger than three years, the diapering surface must be a changing table or countertop designated for changing and that the diapering surface must be used only for diapering or cleaning children. Discussed requirements for background checks and sworn disclosure statements. Discussed requirement for allergies.

Comments:

An unannounced, on-site monitoring inspection was initiated on 5/19/2025 and completed on 5/19/2025, as a part of the licensure period. The on-site inspection began at 10:38am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 118 children present and 28 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-4
Description:	Prior to hire, the center is required to request background checks from any state in which the individual has resided in the preceding five years. The center requested a search of the child abuse and neglect registry from one state for Staff #10 two days after their date of hire.
Plan of Correction:	Will request out of state background checks prior to hire date going forward. Staff #10 did not inform us that she worked out of state within the past 5 years until she started.
Standard #:	8VAC20-780-240-E
Description:	REPEAT VIOLATION: Within 30 days of the first day of employment, staff must complete orientation training in first aid. Staff #2, who is working with children has not yet completed this course which is overdue by six months and nineteen days.
Plan of Correction:	Will provide the required first aid knowledge training and schedule the staff member for First Aid certification. Staff #2 had current CPR certification when she washired; it is still current.
