An unannounced, on-site monitoring inspection was initiated and completed on May 15, 2025. The on-site inspection began at 11:30am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 101 children present and 13 staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain required information.
Child #1, 2, 3, 4 and 5's records did not contain documentation of parents' work phone numbers and place of employment, and previous child day cares and schools attended.
Child #1 and 3's records did not contain documentation of two emergency contacts. Child #2's record did not contain documentation of the city and state for both emergency contacts listed.Plan of Correction: During the initial enrollment process, ESERA manager will ensure that all forms are fully completed with required signatures.
Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center shall be maintained in a clean, safe and operable condition.
In the Patchwork Classroom (children ages 3-5 years), ants were observed crawling on the counter and inside of the cabinet next to the sink.Plan of Correction: The facilities manager will continue to report on all unsafe conditions to Fairfax County for extermination services.
Standard #: 8VAC20-780-280-B Description: Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
A container of hand sanitizer was found sitting on top of the children's shelf in the open closet area.Plan of Correction: The health manager will thoroughly monitor the areas where children occupy to ensure that they are free of dangerous substances.
Standard #: 8VAC20-780-510-I Description: The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center has obtained written authorization from a parent or guardian.
Child #2's epinephrine auto-injector had a corresponding authorization form that did not contain parents signature.Plan of Correction: The health manager will verify that all medical forms are completed properly prior to receiving medication for children.
