An unannounced monitoring inspection was initiated on 05/13/2025 and completed on 05/19/2025 with local agencies, in response to a self-report received by the licensing office on 05/08/2025 relating to supervision. The inspector reviewed 2 children?s records on-site and 3 staff records on-site and electronically on 05/19/2025. This inspection included document review and interviews.
Though not a part of the original self-report, violations that were not reported were found related to staff training and supervision are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-240-A Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff 3 has not yet completed this course, which is overdue by over 1 year.Plan of Correction: Training has been obtained.
Standard #: 8VAC20-780-350-B-4 Description: The required ratio for children 3 years old up to school age eligible is one staff per ten children.
On 05/08/2025, Staff 1 was alone supervising 11 children (all 3 years old) for 2 to 4 minutes.Plan of Correction: Staff will alert administrators when they have reached the ratio maximum to request additional staff.
