During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussion on safe sleep and medications.

Comments:
An unannounced, on-site renewal inspection was initiated on May 9, 2025 and completed on May 15, 2025. The on-site inspection began at 11:40 pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 120 children present and 26 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on May 9, 2025.

This inspection included document review, tour of the facility, interviews, and observations.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: The center is required ensuring that the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center's police and procedurs were not followed for allegeries and asthma medication authorization were current.

Plan of Correction: Tierra Encantada will continue to oversee and ensure that all medical and allergy-related forms remain current and compliant.

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child A was onsite, but the medication authorization had expired over 10 months ago. Medication for Child B was onsite, but the medication authorization had expired three years ago. Medication for Child C was onsite, but the medication authorization had expired over 10 days. The medication was administered five times during the expired period. The authorization for Child A, B and C had not been renewed and the medication was not returned or disposed of as required.

Plan of Correction: Tierra Encantada has taken immediate steps to ensure the safety and compliance of all children?s medical and emergency documentation. Families were promptly contacted via email and asked to update their FARE (Food Allergy Research & Education) forms and Medical Authorization Forms without delay.
For children with expired forms, their families have submitted written letters to the administrative team acknowledging the importance of updated documentation and outlining their agreement with the school?s emergency response plan in case of a medical incident.
Additionally, all staff members have received updated training on medical authorization procedures. This training was conducted during the Staff In-Service Day on May 16, 2025. Teachers have signed off on the completion of this training and committed to remaining vigilant in monitoring and maintaining all required documentation for medications.
