An unannounced, on-site monitoring inspection was initiated and completed on 5/21/2025. The on-site inspection began at 12:15pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 74 children present with 12 staff. The inspector reviewed 10 children?s records and 5 staff records on-site on 5/21/2025. This inspection included: document
review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-B-1 Description: The required staff to child ratio for birth up to 16 months is 1:4 (staff:children).
There were 10 children with 2 staff in a group where Child A in the group was under 16 months of age, requiring the ratio to be 1:4. The classroom was over the ratio from approximately 12:15pm-1:25pm, when a third staff member was added to the classroom.Plan of Correction: Another staff member was added to the classroom during the inspection to correct the ratio. We will make sure we follow a 1:4 ratio when children are under 16 months old.
