During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 5-19-2025 and completed on 5-22-2025. The onsite inspection began at 1:45pm and ended at 4:45pm. The inspector reviewed compliance in the areas listed above. There were 46 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and
violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/30/2025.
A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-J
Description: The center is required to update at least annually the injury prevention procedures based on documentation of injuries and a review of the activities and services. There was not an updated injury plan available for review.

Plan of Correction: The center already had the updated correction plan

Standard #: 8VAC20-780-70
Description: Repeat Violation
The provider shall keep for each staff person the following information for each staff record. Staff #3 who has been working for the past 13 months and was missing information related to any health issues that may interfere with the job and Staff #4 was missing a name, address, and telephone number of a person notified in an emergency.

Plan of Correction: Director will update information for staff 3

Standard #: 8VAC20-780-80-A
Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
1.In Room A there were 12 on the attendance list and 10 in care
2. In Room B there were 4 on the attendance list and 7 in care
3. In Room D there were 3 on the attendance list and 10in care

Plan of Correction: Director will get with each staff about arrival and departure of the children

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
1.On the preschool playground there was a picnic table with splintered wood, chipped and peeling paint.
2 On the preschool playground there were two sections of exposed sharp concrete sticking out of the ground near the chalk wall.
3. In Room F there was a book shelf and diaper changing table that each had areas of splintered wood, chipped and peeling paint.
4. In Room A there was a diaper changing pad and table that had areas of dirt. Wood chips were observed under the diaper pad and the dirt on the diaper pad and table came off when wiped with a paper towel.

Plan of Correction: The picnic table will be painted 6-7-25
Look into having concrete removed 6-8-2025
The bookshelf in room F will. be pained 5-22-2025
The are of the diaper pad and table will be sanitized and clean after each use. 5-19-2025

Standard #: 8VAC20-780-330-B
Description: Repeat Violation
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus. On the Two?s playground the resilient surfacing at the base of the blue slide chute measured between 4-5 inches and a minimum of 6 inches is required.

Plan of Correction: Mulch will be delivered on 6-6-2025

Standard #: 8VAC20-780-440-F
Description: Rest mats used for napping shall have cushioning and be sanitized on all sides weekly. A mat used by a child in Room B for napping was torn around the edges, had exposed foam and was no longer nonabsorbent and cannot be sanitized.

Plan of Correction: Staff will make sure mats are in good condition so they can get a replacement and will be sanitized

Standard #: 8VAC20-780-500-A
Description: Staff shall wash their hands with soap and running water before preparing or serving food or beverages. Staff #3 in Room A did not wash their hands before preparing and serving snack.

Plan of Correction: Talked with staff to make sure the children are washing their hands before and after they eat

Standard #: 8VAC20-780-500-A
Description: Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Children in Room A and Room D were observed eating snack and they did not was their hands after eating.

Plan of Correction: Moving forward staff hands are washed and have gloves on

Standard #: 8VAC20-780-500-B
Description: The diapering area shall have a sink with running warm water not to exceed 120?F. The mobile sink designated for diaper changing had run out of water and was not available for children and staff to wash their hands after diapering.

Plan of Correction: Mobile sink will be filled up everyday at e end of the day

Standard #: 8VAC20-780-550-P
Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made on the day of occurrence. Five injury reports were reviewed, and they did not include the date and time when parents were notified; future action to prevent recurrence of the injury and documentation on how parents were notified.

Plan of Correction: The center has updated the injury reports to the licensing form online

Standard #: 8VAC20-780-560-M
Description: Children shall not be allowed to drink or eat while walking around. Two children were observed walking around eating crackers in Room B and two children were observed walking around drinking in Room A.

Plan of Correction: Staff will make sure children are sitting while they are eating and drinking
