Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 5/12/25. The on-site inspection began at 4:00pm and ended at 5:40pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 17 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/30/25. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.011-F
Description:	Repeat Violation The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 12/16/24, were not posted in a conspicuous place on the premises.
Plan of Correction:	The facility responded: On 05/13/25, the inspection summary dated 12/16/24 was printed and posted in a clear visible location.
Standard #:	8VAC20-780-60-A
Description:	Repeat Violation Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for child #2, child #3, child #4 and child #5 did not contain all of the required information.
Plan of Correction:	The facility responded: A full review of all enrolled children's files was conducted on 5/14/25. Any missing or outdated documents were collected and files were brought into full compliance.
Standard #:	8VAC20-780-70
Description:	Repeat Violation Staff records shall be kept for each staff person and shall contain all the required information. The record for staff #1 did not contain the date of employment.
Plan of Correction:	The facility responded: As of 05/13/25, the missing date of employment has been obtained and added to Staff #1's personnel record.
Standard #:	8VAC20-780-240-A
Description:	Repeat Violation Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #2, who was working with children, had not yet completed the course which was overdue by 4 months.
Plan of Correction:	The facility responded: Staff #2 completed the 10 hour orientation training on 5/13/25.
Standard #:	8VAC20-780-340-C
Description:	During the hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee. Staff #1 was the only staff member on the premises with 6 children after 5:15pm.
Plan of Correction:	The facility responded: A backup list of on-call staff has been created to cover any staff absences or late departures. The directors will review the attendance and staffing logs daily to verify that proper coverage is maintained throughout the day.
Standard #:	8VAC20-780-530-A
Description:	At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid. Staff #1 was alone with a group of 6 school age children and was not certified in CPR or first aid.
Plan of Correction:	The facility responded: Staff #1 completed CPR/First Aide certification on 5/14/25.
Standard #:	8VAC20-780-530-C
Description:	There shall be at least two staff members present who have current cardiopulmonary resuscitation (CPR) and current first aide certification during the center's hours of operation, and whenever children are in care. On 5/12/25 the facility did not have two staff present with current CPR and first aide certification after 5:15pm.
Plan of Correction:	The facility responded: As of 05/14/25, the schedule has been revised to ensure that two certified staff members are present during all hours of child care.
