An unannounced monitoring inspection was initiated and completed on 5/12/2025 in response to a self-report received by the licensing office on 4/25/2025 relating to forbidden actions. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to forbidden actions and are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-70 Description: Centers are required to maintain a record for each staff person which contains all required information. Staff #1 was identified as the lead teacher in the classroom. The record for Staff #1 did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position. Plan of Correction: Supporting documents will be added to Staff's record.
