8VAC20-780 SPECIAL SERVICES.
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated and completed on 5/22/2025 in response to a complaint received by the licensing office on 5/19/2025 relating to allegations of transportation. The inspector did not review staff or children?s records. This inspection included document review and interviews.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
The complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/30/2025. A POC submitted after this date will not appear on the public website.

Administration at the center stated there was no information posted about the trip?s event or location at the center.Plan of Correction: Post when children have a fieldtrip and inform parents.
Standard #: 8VAC20-780-580-M Complaint related: Yes Description: If a blanket permission is used instead of a separate written permission, the parent shall be notified of the field trip; and given the opportunity to withdraw their children from the field trip.
There was a blanket permission slip in children?s record but parents were not notified of the fieldtrip or given the opportunity to withdraw.Plan of Correction: Post when children have a fieldtrip and inform parents.
