An unannounced, on-site monitoring inspection was completed on 05/19/2025, as a part of an Intensive Plan of Correction (IPOC). The on-site inspection began at 11:55 am and ended at 1:25 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 2 children?s records and 5 staff records on-site. This inspection included document review.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
