Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on May 6, 2025 and the inspection concluded on May 9, 2025. The on-site inspection began at 11:45 a.m. and ended at 3:45 p.m. The inspector reviewed compliance in the areas listed above. There were 100 children present and 18 staff. The inspector reviewed 10 children?s records and 11 staff records on-site. Additional information was sent electronically on May 8, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-2
Description:	Documentation of a fingerprint based national criminal check shall be obtained for each staff prior to employment. Staff 5 (date of employment 10/30/2024) did not have a fingerprint result on file until 11/04/2024.
Plan of Correction:	Fingerprints will be on file prior to an employee's offer for employment.
Standard #:	22.1-289.035-B-4
Description:	The center is required to do the appropriate background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one previously resided state for Staff 11, who has been employed for five months.
Plan of Correction:	Out of state background checks will be submitted prior to the first day of employment.
Standard #:	8VAC20-780-130-A
Description:	The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 4, Child 7, and Child 10 did not have documentation of an immunization record prior to attending the center
Plan of Correction:	Immunizations shall be obtained prior to the child's first day of attendance.
Standard #:	8VAC20-780-140-A
Description:	Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 4, attended for 8 months prior to receipt of a physical and Child 10, attended for 3 months prior to receipt of a physical.
Plan of Correction:	Physicals will be obtained within 30 days of the child's first day of attendance.

Description:	The center shall provide to the licensing representative an annual health inspection report. The last documented health inspection was conducted 12/12/2023.
Plan of Correction:	The health department inspection will be requested.
