Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site 05/22/2025 monitoring inspection was initiated and completed on 05/22/2025. The on-site inspection began at 11:45 a.m. and ended at 1:50 p.m. The inspector reviewed compliance in the areas listed above. There were 68 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-70
Description:	Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff 4, who has been employed over 2 months, only had 1 reference on file.
Plan of Correction:	Provider will obtain a second reference.
Standard #:	8VAC20-780-80-A
Description:	For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. At approximately 11:50 a.m. on 05/22/2025, there were 9 children present in the toddler room, but only 8 were marked present on the attendance record.
Plan of Correction:	Staff will document the arrival of each child as it occurs.

Description:	Cribs, cots, rest mats of beds shall be provided for children during the designated rest period. On 05/22/2025, 6 children (all age 5), were observed laying on the floor during the designated rest period.
