An unannounced, on-site monitoring inspection was initiated on 05/22/2025 and completed on 05/22/2025. The on-site inspection began at 8:58 am and ended at 10:10 am. The inspector reviewed compliance in the areas listed above.There were 18 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-240-B Description: Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff 3 did not have documentation of orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities. Staff 3 has been employed with the program for over one month.Plan of Correction: Did review but I did not get staff to sign documentation. I will make sure that I have the staff sign the paperwork after orientation.
