Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

This inspection report was amended on June 17, 2025. An unannounced, on-site monitoring inspection was initiated and completed on May 21, 2025. The on-site inspection began at 10:30 am and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 114 children present and 14 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included a tour of the facility, interviews, observations, and document review. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-160-A
Description:	Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. Staff #3 had no documentation and had been employed got nine months.
Plan of Correction:	Staff Member #3 completed the TB screening with our school nurse upon hire and was cleared to begin employment. Unfortunately, the nurse mistakenly missed the signature line. As this nurse is no longer employed at our school, we could not backdate the form to include the medical professional's signature, which is the one who completed the screening. The school immediately had our on-site nurse recomplete and sign the form. Employee #3 was rescreened on 5/21/25 and remains cleared to work in our program per licensing's standards. Our school nurse and administration will continue to audit forms for completion and accuracy.
Standard #:	8VAC20-780-260-B
Description:	The center shall obtain annual approval from the health department for meeting requirements for water supply and sewage disposal system. The center's environmental sanitation inspection was not completed.
Plan of Correction:	The school expects a visit from the Health Department within the next month. As these visits are unannounced, we do not have the exact date. The school administration and the school cook all have copies of the required form. The Chesterfield Health Department inspector will be given the form to complete at our next inspection, and the school will provide the completed form to our licensor upon receipt. The administration will continue monitoring and auditing the completion of the necessary form to remain compliant.
