During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated on 5/23/25 and completed on 5/29/25 in response to a complaint received by the licensing office on 4/22/2025 relating to allegations of lack of supervision, and a self report of biting incidents. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, interviews, observations (this would include observations of photos, recordings, etc.), and

The preponderance of evidence gathered during the investigation supports the allegation of lack of supervision; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were reported as a self report (biting) were found related to supervision and are documented on the violation notice issued to the program.

Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: name, address and phone number for emergency contacts.

Child 1 did not have documentation of an address for one of their emergency contacts.

Complaint related: Yes
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

1) Child 1 was in care when they chipped their tooth, loosing half of their front tooth, and none of the teachers supervising them knew how or when it happened.
2) Child 2 has 59 documented bites to other children in the facility.
3) Child 3 has received a total of 35 bites from other children in care.

Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: date and time parents/guardians were notified.

7 injury reports did not have documentation of the time parents/guardians were notified of the injuries.
