During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 05/15/2025. The on-site inspection began at 12:00 p.m. and ended at 1:15 p.m. The inspector reviewed compliance in the areas listed above. There were 14 children present and 2 staff. The inspector reviewed eight children?s records and two staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/05/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-70 Description: The center shall maintain and keep at the center a separate record for staff that contains all of the required information.
The record for Staff A, employed for more than nine months, was missing the following required components: 1.) Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.Plan of Correction: Review staff records and submit to inspector proof of staff member's education, certification, and experience.
Standard #: 8VAC20-780-240-A Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff A, who is working with children, has not yet completed this course which is overdue by more than seven months.Plan of Correction: Provide documentation of completion to inspector as proof of completion of training.
